Demonstration of the importance of microvascular integrity in myocardial salvage has focused intervention on the target lesion and, importantly, on the prevention of distal embolization of thrombus.1 Distal emboli 'plug' the microvasculature and produce spasm and a local inflammatory reaction that can complicate recovery further. There are many cases where inadequate myocardial perfusion is undetected but could be detectable by delayed resolution of ST-segment elevation or by decrements of abnormal MB grade.2
