Imatinib mesylate (STI571, Glivec) is a protein-tyrosine kinase inhibitor that inhibits the bcr-abl tyrosine kinase, created by the Philadelphia translocation, t(9;22)(q34;q11) (Ph+) of chronic myeloid leukemia (CML). In the first studies, the drug appears to be very effective and generally well tolerated,1,2,3 but as it is only recently introduced, few data are available about long-term effects, specifically the possibility of secondary malignancies. Recently, several patients have been noticed to develop clonal karyotype abnormalities in Philadelphia translocation negative cells (Ph−).4,5,6,7
