Breakthrough cytomegalovirus pneumonia in hematopoietic stem cell transplant recipient on letermovir prophylaxis

Letermovir, an antiviral agent that inhibits Cytomegalovirus (CMV) replication by binding to components of the terminase complex (UL51, UL56, or both), was Food and Drug Administration approved for CMV prophylaxis in hematopoietic stem cell transplant (HSCT) patients in November 2017 [1, 2]. To date, a single case of breakthrough CMV viremia on letermovir prophylactic treatment has been published, conferred by a UL56 V236M mutation, and occurring on a daily dose of 60 mg [3]. We report a case of breakthrough CMV disease in an HSCT patient on letermovir prophylaxis conferred by a UL56 C325F mutation [4].
