We want to thank Prashanth et al.1 for their comments regarding our article about using very high frequencies with very low lung volumes during high-frequency oscillatory ventilation (HFOV) to protect the immature lung.

Our study2 focused on the feasibility of using the lowest high-frequency tidal volume (VThf) possible combined to very high frequencies during HFOV, showing that CO2 removal is maintained similar to large tidal volume (VT) HFOV. The idea to decrease the VThf is based in the deleterious effect of large VT on the ventilation-induced lung injury (VILI) generation in the immature lung. The target population was mostly preterm infants, but we also wanted to demonstrate this feasibility of using the lowest VThf possible in larger term newborns, which are also susceptible to develop VILI. As demonstrated in our study, term infants were also ventilated using lower VThf and the CO2 removal was maintained using higher frequencies than normally are used in this population during HFOV. Thus a potential protective role of this new modality is now proposed when the volume guarantee is used also in term newborns infant.
