We agree with Dr Razak1 that data about inadvertent hypocarbia, mean tidal volumes, and stages of necrotizing enterocolitis (NEC) might be relevant to the mechanisms by which volume-targeted ventilation (as compared to pressure-limited ventilation) could improve neurodevelopmental outcome.2 Limited resources precluded our collecting data on hypocarbia and tidal volume. As we acknowledged in our paper, our approach to the data on NEC was limited by our inclusion of stage 1, which is not proven NEC. Although data on these three factors might have been helpful in understanding our conclusions about the relationship of volume-targeted ventilation to the primary outcome we studied, these factors cannot be viewed as confounding variables, since they occurred after initiation of ventilation.
