Is anterior placentation per se a risk for cesarean section-related morbidity in placenta previa?

Is anterior placentation a risk for cesarean section (CS)-related morbidity in placenta previa? Young et al.1 showed increased surgery-related morbidity in anterior placentation in this condition. Anterior placentation, compared with posterior placentation, increased the incidences of (1) blood transfusion, (2) hysterectomy and (3) the amount of estimated blood loss. Importantly, (1) and (2) were true not only for repeat CS but also for primary CS. However, this study did not answer the question whether anterior placentation per se is the cause of these increases.

Obstetricians may become more cautious for anterior than posterior placentation at repeat CS for placenta previa. This is mainly because anterior placenta, with the placenta covering the previous CS scar, may become ‘accreta (accreta, increata, percreta)’ at the site: in short, obstetricians are concerned about the presence of ‘accreta’ at anterior placentation.
