Since the publication of this issue, an error was noticed on page 292 under the section Conditioning regimen. The dose and usage of Simustine (Me-CCNU) are not correct.
The phrase ‘Me-CCNU (250 mg/kg, i.v.) on day −3’ should read as ‘Me-CCNU 250 mg/m2 orally once on day −3’.
