High-dose chemotherapy with autologous PDSCT as post-remission therapy for AML

This is a response to Dr Kim's correspondence1 regarding our clinical study of high-dose chemotherapy facilitated by autologous peripheral blood stem cell transplantation (auto-PBSCT) as post-remission therapy for AML in first complete remission (CR 1). Our conditioning regimen consisted of G-CSF administration at doses of 5 μg/kg on days −12 to −8, 10 μg/kg on days −7 to −6 and 20 μg/kg on days −5 and −4 in combination with 24-h continuous infusion of low-dose Ara-C (100 mg/m2 on days −10 to −4) followed by high-dose Ara-C (3 g/m2 i.v., every 12 h on days −4 to −3) in addition to high-dose busulfan (4 mg/kg orally on days −9 to −6) and etoposide (20 mg/kg i.v. on days −5 and −4). This regimen is referred to as G-CSF combined BEA. The 5-year event-free survival was expected to be 74.5% in 16 patients with AML in CR1, who received G-CSF combined BEA with auto-PBSCT.2

In addition to this preliminary but encouraging result, we also reported our subsequent results.3 Sixty patients with AML were treated with the G-CSF–combined BEA regimen followed by auto-PBSCT. The 5-year probability of disease-free survival and 3-year probability of relapse were 70.7% and 21.4% in 42 patients transplanted during CR1, respectively. Oral mucositis and gastrointestinal symptoms were common toxicities related to G-CSF-combined BEA. Oral toxicity greater than grade II was observed in 23% of the patients. There was no early transplant-related mortality (TRM) within 100 days in patients transplanted in CR1. The median day to recovery of granulocytes >500/μl was 15 (range, 8–44), and the median time to the last platelet transfusion was 24 days (range 0–>180).
